Instruction for using our Online Registration Form

Thank you for using our online "Registration Form".

From the selections to your left, you can choose to print out a "Single Competitor","3 Man Team Fighting" or a "Family Member" registration form.
Just print, fill out, and mail along with your payment before our pre-registration deadline to receive our pre-registration savings.

If the competitor is a minor, please have a parent or legal guardian sign their registration form for the required
"Hold Harmless Waiver/Release"

Pre-Registration

To take advantage of our Pre-Registration discount, all registration forms and their payments must be received at the address above, postmarked on or before the pre-registration deadline.

The pre-registration deadline for the "Battle of the Champions" II tournament is:
Saturday, October 17, 2009.

Family Rate

To take advantage of our Family Discount, each competing family member must fill out a registration form with all minor competitors having their registration form signed by their parent or legal guardian.

The cost is per family member.

All registering family members must be immediate family members only, NOT extended family (uncles, aunts, cousins etc). Family members must be under the age of 18 and living in the same household.
